2) Appointments, Cancellations & Late Arrivals

• Please arrive 10 minutes early to complete any forms.
• We request at least 24 hours notice to cancel or reschedule. Missed or late-cancellation appointments may be subject to a fee.
• Late arrivals may need to be rescheduled to respect other patients’ time.

3) Payments, Insurance & Financing

• Payment is due at the time services are rendered unless prior arrangements are made.
• As a courtesy, we may assist with insurance claims; however, you are responsible for all fees not covered by your plan.
• Quotes are good-faith estimates and may change based on clinical findings or treatment choices.

4) Medical Information & Consent

• You agree to provide accurate and complete medical/dental history and to update us of any changes.
• Treatment is performed only after informed consent is obtained. You may ask questions at any time.

5) Treatment Outcomes & Warranties

Dentistry involves biological variability; therefore, outcomes can vary by patient. No guarantees or warranties are expressed or implied unless provided in writing for a specific service.

6) Communications

• By providing your email or phone number, you consent to receive appointment reminders and important updates. You may opt out of marketing messages at any time.
• Email and text may not be fully secure. Avoid sending highly sensitive information via these channels.

7) Website Use
